1. Have a question or need help with your card? Please send an email to info@ugca.au. Please provide the following information to help us help you as quickly as possible
   • Card Proxy ID (which is the 9 characters following the PN on the back of your card)
   • The name of the person or entity that gave you the card
   • The address to which the card was originally sent
   • Your preferred email and/or contact number for us to contact you on
   • any attachments to help with the enquiry
